I chose to implement the 'Decision Log' form on my confluence site. When I log an item, I can choose a status of Not Started, In Progress or Decided. When I attempt to edit an In Progress Item and change status to Decided, it doesn't appear I can choose from the Status List and if I type Decided in the Status field, it does not format and record properly

I'm Elena from Stiltsoft. For your purposes, you can use our Handy Macros for Confluence.
It allows you to create multiple status sets to suit your workflow. You can always switch statuses in the page view mode, which makes your Confluence pages dynamic.
You can also make use of the combination of the page properties and page properties report macros to ensure consistency of your information among pages.
For example, here we put a table with statuses in the page properties macro:
Later on, you can click the status name to change it on the go from your personal customizable set:
If you need the info from this page to be present on the other Confluence page, you can insert the page properties report macro there:
This report renders the information from the specified pages with all the statuses being dynamic. So you can switch them here in the page view mode and be sure that they are updated in the source table as well.

Thank you - yes - I did see this option originally but I must have done something at some point on the specific item I was working on to 'deactivate' it because the option was no longer available. I'm on the right track now - thanks!

I hope you have Page Properties macro added on your page with a table inside it and added the custom Status label in the column. So, when you click on it, it will let you select a color and text to display. See example below -
