I'd like to get an email or some other prompt to review stale documents. E.g., the ability to flag a document (and its children) with something like "prompt me to review if there have been no edits for X days".
Is this a thing?
If you are only working with out of the box features your options are limited.
But to make this task easier, you could try using an advanced CQL search to identify content matching your criteria.
There is a free app called "CQL Search" in the marketplace that can help with this.

There are some content management add-ons that might have this functionality or similar, although they may give you a lot more than you require - e.g. workflow or drafting. I can't tell you for sure, but I'd start with options like Comala Document Management, Scroll Documents for Confluence, and Workflows for Confluence.
