I got a page in confluence cloud with some forms to be filled by the users. My idea is to:
Create a template form and insert submit option. And after submitting can this directly create a ticket in JIRA.
I know a confluence page can be mentioned in JIRA issue using Link option in JIRA and Jira issue can be mentioned in confluence using the macros. But is there any way that creating a template form in confluence and insert submit button in the form. And by submitting the form, it should automatically create a ticket.

There are two simple ways to create issues on a Confluence page:

Thank you. But unfortunately I am looking for some other things. I know may be my question is silly. But I just wanted to know if I create a page with forms and submit buttons ( I got that from Forms for Confluence Plug-in). Instead of creating a new issue in JIRA. If I can directly fill the form with data as shown below;
Issue type:
Project:
Summary:
*Create a Task*
Above is the form I created from the plug-in but I am unable to create a task directly with automatically filling the information in confluence page and that goes directly to JIRA as a ticket. Just checking if that is possible.

I worked with JIRA macro. But using this macro we can just mention the issue in a confluence page but the changes in the page cannot be reflected directly in the ticket. we link confluence page in JIRA ticket and thus the page can be seen. But the direct import of data from a confluence page to JIRA Ticket is what I am looking for. May be this is not available.

In default Confluence, there isn't an option available to achieve exactly what you need.
However, after further research, I could find this documentation from the vendor of the app you've mentioned:
As per the doc, it seems to be possible to achieve this using Forms for Confluence with another app called ScriptRunner for Jira.

I achieved that using Jirassimo add-on, by which I can include a form in confluence and if the user fill and submit the form a ticket will be generated in the linked project.

I actually did that using two ways, one with Issue collectors in Jira I created a form and used the HTML from it. And I have installed HTML reader add-on in confluence page and pasted the HTML script there and published the page. Then I got the required form there and If I submit the form then the ticket got generated in the project I integrated the issue collector. The second way is using Jirassimo Intake form and integrated it with confluence and I got the results.
I also tried Script runner, but the customization is not same as the server version of the add-on. And it didn't satisfy my requirements.
