Why does the comala workflow resets after getting reviewed?

A document was sent for review by four members. After three members reviewed and approved it, the workflow state changed back to draft. Hence the fourth member was not able to review or approve.

Most importantly, the version history remains untouched or unchanged.
This problem persists every Monday and Friday of the week.

I have raised ticket. JSON editor access I don't have. However the workflow screenshot has been attached.
