Submit a Request on Confluence

Hi,

I am looking for information on how to submit a request on the confluence page and track it through JIRA. example: User can submit a request to the team asking to update fields.

How it can be done in Confluence, do we have any forms ?

Welcome to Atlassian community. 

You can do with many apps available in the atlassian market place. One recommendation would be to check out on ConiForms.

But do not restrict yourself to this one and explore all the other options available in market place. Here is the list.
