Tab disappears when updates are made

One of the tabs disappeared when changes were made to our confluence page. It seems like multiple times we made updates, the tab disappeared, and we had to roll back the change. We did this about 5 times. Any ideas on what would cause that?

The macro used was UI Tabs. (See first image)

The second image shows the 2 tabs before any edits were made. The third image shows only one tab after edits were made. 

A user went to EDIT the first tab and when she did she couldn't find it to EDIT.  She cancelled and the tab was gone (third image).  We tried with several peeps and had the same experience.  So we went to the Page History and reverted back several edits and it was back.  We attempted to make an edit again and the tab disappeared again.
