Yes, it is possible, with the help of our apps Run Self-Service Reports for Confluence and 'SQL for Confluence (Pro Edition)' we can achieve this requirement.
Users can enter some values in a template and based on the values, we can run a SQL command to show the records from a table. For this, we have created a user form with 2 text fields (id, name). When a user gives proper values to id, the name then only we can see the corresponding record. Please see the below screenshot for the same.
