User activity showing up as from other user

We have a user who's activity (e.g., page edits) are showing up as coming from a different user. 

Any ideas what might be causing this?

I'm assuming this is in Confluence and you've already checked that the person is logged into to the correct credentials.

Is their profile information just wrong by any chance? I had a similar issue and it ended up being SOMEHOW the account was created with blank information so they were showing up as "unknown user".

It wasn't an "Unknown User" issue. They were logging in from a shared account on a VM, so I suspect they were logged in as the other user when they performed the actions. We haven't seen the problem again recently.
