We are encoutering a very strange behavior of Confluence that every time a user tries to log in Confluence, they are asked to enter CAPTCHA even though the system is configured to only show CAPTCHA after 3 failed login attempts. The issue only occurred once and the behavior disappearred after restarting Confluence. We want to know if there is anyone else in the community also has encountered this issue and know the possible root cause of it?
We also run into the same issue. We think we have found the cause and an easier workaround than restarting the server. Atlassian support have created an issue for this case:

At the moment, if you have a public Confluence there is no easy way to stop anonymous users from being able to put it back into this state.
We've just seen the exactly the same thing here. Will log a support call for it.
