how to create a Form with email cc to self/current login user?

Confluence : I want to create a Form, with Email to recipient#1, reciepient#2 and cc to self. So whenever users submits a form, email should be sent to recipient#1, reciepient#2 and cc to himself. How can I configure this (mail cc to himself)?
