My server died and requires a new Confluence installation. I have a mysql database backup but cannot get the new Confluence instance to link to that database. What are the steps to link the two?

If you have a file system backup of the Confluence Home directory (defined in confluence/WEB-INF/classes/confluence-init.properties) the confluence.cfg.xml file in the root of that folder will "tell" Confluence how to connect to the MySQL database. You will also need to put the MySQL driver in the <installation-directory>/confluence/WEB-INF/lib directory as described in Database Setup For MySQL .
If you don't have a file system back-up, it's a shame because attachments are usually stored in the Home directory but you can get most of your content back by installing a "dummy" instance and then changing the JDBC connection details to the back-up of Production.
