The day portion of the date in which a page was updated is incorrect

I just noticed that all of the dates shown for page updates are incorrect.  The day portion of the date is three digits.  As an example, changes made yesterday (4/21/2016) are being displayed as "04/111/2016".  What is causing this and how do I fix it?

Under General Configuration in the Admin area, check the date format.

Date Time Format is currently set at "MMM dd, yyyy HH:mm"

Date Format is currently set at "MM/DD/YYYY"

Based on this I would say that the day should always be two digits.
