Trying to Integrate Jira and Confluence, but having issues

Jira is onDemand

Confluence we are hosting ourselves.

When I go to one or the other, through Application Link, it's not able to auto-detect the information.

Is there something else i need to do?

While this is not officially available for other than FishEye and Curcible, but I have heard some people that got it to work. First thing is to check which port your hosted Confluence is running on, it must run on port 80 or 443 to be picked up by OnDemand.
