Word Export add an Header

Hi 

I hade a little question. Could someone help me, how could i insert some Header information by a word export. 

I want to have this information in the header when i export a confluence page in a word file:

• Pagename
• Version Number
• Last change Date

 i have unfortunately no idee how i can insert this i a header. maybe someone can help me with a manual.
