Does anyone know of a way to create on line survey forms - something like "Survey Monkey".
I want all the responses to be collected within Confluence so that it can be used/referenced in various ways.
Ideally I would like to have a "common section" where if we are running many different surveys associated with different Spaces, the respondent can select their company name from previous surveys to automatically fill in the general data about their company.
The survey tool needs to be able to be completed by general public access as well as logged in users but can only be created and edited by certain Confluenvce Users within each Space. It would need to be able to select whether it is for Confluence Users only or also for public - perhaps that is just a question of where teh survey form is "published" in different Spaces
The Google Docs in Confluence plugin (from AppFusions) provides the ability to embed Google Forms into Confluence.

Wufoo is similar to "Survey Monkey" and the best part is that it can be integrated with Confluence. Forms created in Wufoo can be embedded in Confluence as described in http://wufoo.com/partners/confluence/
