We have some very specific fields we'd like to have contributors populate when creating an idea. How do I add custom fields to the create screen?

You should be able to manage this through each JPD project's Settings >> Features >> Create ideas tab:
As you can see above, I've select the Simplified Template View which is a list view in my JPD, and the fields selected in this view will appear in the Create Idea form when a contributor is creating an idea.
I've also required the fields to mandatory so the contributor will need to enter values for each of these in order for an idea to be created:
