How can I create a form that captures an initiative and then add that initiative to a list of initiatives in order to have a view of the pipeline for the Executive team to understand projects inflight, future projects.

When you create a new Jira Product Discovery project, you can also create a form for this project, and then share a link to this form. That way you'd be able to collect initiatives, then prioritize them according to any data you want, and create roadmaps.
So first, I'd suggest to check this demo, or look at this getting started guide to be more familiar with Jira Product Discovery capabilities.
To create a form, you can go under Project settings > Features > Create ideas

A business project in Jira would be more suitable for your use case, because it has a specific Forms feature that you can share with your stakeholders and capture their response.You can limit it only for the users within that project to fill out the form.You can also define to which work item type the form will be connected to.
