I'm a Jira Administrator and I'm setting up the Product Discovery for use in my organization. I got a set of custom fields that I need to display on the Create Ideas form and I also need to hide the out-of-box fields that are displayed on the form. I can only see a Form preview but I cannot edit to remove fields.
In JPD, the create dialog is based on the fields present within a specified "View". In order to customize the create dialog how you want, you would need to create a view containing the specific field you want presented on it. The "Form preview", as you pointed out, is not editable. The fields will be presented on the create dialog in the order in which they exist on the view. The Summary & Description field will always be present and will always be the first 2 fields.
The "Description templates" section can be used to specify what is contained within the Description field on the create dialog. This can be cleared out by creating a template that doesn't contain anything in the field and applying that.

NOTE: This only applies when the "Create" button is clicked when in the specific JPD project. If the "Create" button is clicked while navigated somewhere else within Jira (i.e. any other project) and then the specified JPD project is selected in the project list, the create dialog will show ALL fields contained within the JPD project.
There is also the ability to create ideas directly from within a view. In this scenario only the Summary can be set by the user. All other fields would need to be set from the "View" or the "Issue" view after the Idea has been created. No field specifications can be made for this form of Idea creation.
