About Activity Field under a Ticket?

Q1. How I can set Activity Filed?

I want to set activity Filed Like below.  Please help me about these issues. 

Q2. I am using Filter Gadget for Report. But problem is that, I want to see 'Task plan Date' Filed in a Date and Time. But its showing in a Days. I want to set these issues. Please help. I have attached image below:

1. No, you will not be able to alter the Activity section of the issue screen.
