Add Forms Button When Creating An Issue

I am testing out Proforma and am having an issue getting the 'add forms' button to show up on the creation of the issue itself.  

After creating the issue I can go back into it, click the ... dropdown and add forms but would like the option available on issue creation.

Hello Ryan,

As per the existing ProForma documentation, it seems forms can be embedded to a previously existing JIRA issue - that is, the option to add forms would be available after creating the issue itself, and you can add them later as soon as the issue is created, but not at the same time as you submit the issue.
