Is there a way to add a note in the description field?
For example, to help the team, we want to add the following reminder/guidance when they are entering their descriptions:
"As a [role], I want to [action] so that [outcome]."

You can also add information about format in the description available under text field. This text can help your users: read documentation.
If this text is a little bit longer or need some formatting, I can recommend you using app for templating. For example, Issue Templates Agent created by my team. With this app, you will be able to create template and all values from template will automatically fill fields on the Create issue screen.

What type of project are you using? Get that information from the Type column on the View All Projects page under the Projects menu.
If the project is Team Managed the the Project Administrator can set a default value for the Description field.
If you are working with a Company Managed project you cannot set a default value for the Description field. There is a work around using a custom field that you can set a default value for, adding that field to the Create Issue screen, then using Automation to copy the data from that field to the native Description field after the issue is created. If you are interested in that method, I can provide links to other posts in the community that provide the details.
