Hi @Munir Patel - To add a form to an issue, you can scroll down to the forms section and click Add Form. A dropdown menu will show the available forms. Pick the one you want and click Add.

Hi Pete, thank you for responding. Can adding FORMs be automated for specific TASKs.
Example:
- When a TASK1 is moved from backlog to say "Assigned" status then add FORM A.
- When TASK2 is moved from backlog to say "Assigned" then add FORM B.
FORM Rule allows the FORM to be added to Tasks. But it adds the FORM to all tasks. An extra condition under the rule to allow to select a Task would really help.

Hi Munir - will TASK1 and TASK2 always be unique or do you have recurring tasks?
I'm not aware of a way to automate or select for a specific task.
