Are there any examples of how to create a Jira ticket from Google Forms?

Hello:

Let's say I have a Google Form.  When I click the submit button, it would be great if that could somehow automatically create a Jira task.  I'd also like the info in the 'description' text box on the form to go into the 'description' field in the Jira ticket.

Is that possible?

Are there any examples of how to do that -- my Google searches aren't yielding much results.

Alternatively you can usr Jira Issue Collectors feature which allows you to easily embed a Jira feedback form into your own web site. This form is typically accessed by clicking a 'trigger' tab exposed along the edge of pages in your web site.
