Automated workflow: Move ticket to 'Done' -> Fill out a form -> Confluence page

Hi Atlassian Community,

I'm exploring automated workflows to improve the way my team members document the work they've completed. Ideally, I'd like the following workflow:

• Contributor moves a ticket to the 'Done' status.
• Upon doing so, a form automatically pops up asking the contributor to fill out details about the project. 
• Upon pressing 'Submit', the responses of the form populate a templated document in a specific Confluence space. 

Is this possible? If so, would appreciate any points in the right direction. Thank you!
