Automatic workflow transition trigger not working in some cases

I have configured triggers for auto transition of issue status. To be precise, when a Pull Request is raised in Bitbucket, status has to change from one to another.  i.e Coding/Unit-Testing/Integ-Testing ---> Code Review. This happens correctly from Unit-Testing/Integ-Testing statuses, but from Coding--->Code Review it never does.

How to debug using the logs/triggers.. What are the files that I should check?
