I would like to add more fields in my Issue Collector.
May I know if there are other ways we can add specific fields? Do we need to file a request or is there other product purchase we need to do to add a specific field for our forms?

While creating an issue collector choose the Custom option in Issue Collector Form:
Issue Collector is just a javascript program. You can add there anything you want.
You must be a registered user to add a comment. If you've already registered, sign in. Otherwise, register and sign in.
After you create an issue collector you can see the javascript code. You can modify it as much as you want.

I found it very difficult too.
I succeeded (by chance I guess) to add one custom field
There is a refresh issue somewhere.
What I did,
And there, I was able to see my field, which is, in my case "Acceptance Criteria" (see below)
You may not need to do all these steps above.

The link provided by Betsy, which you can find at the bottom of the issue collector page too, is quite useful if you want to define default values.
I never succeeded to launch my "form" via a button though. Nothing happen. I just can with the Prominent / vertical / ... not custom
