Can we create custom Issue actions ?

I would like to create a custom action button for any issue. 

For Example:  I would like to add  a button "Documentation Required" Issue action that creates a new Documentation issue and links it to the current issue. 

Is this possible ?

Yes, with code - web-fragments and web-items created in add-ons can add links to any of the menus, and trigger code to run behind the actions.

I'd do this with the Script Runner now that it can inject web-fragments.
