Can you add an attachment option to a form?

I've created a testing form, but we would like to include the option to add an image attachment.

Just add the Attachment field to your form.

You must be a registered user to add a comment. If you've already registered, sign in. Otherwise, register and sign in.

You will have to use the existing attachment customfied. Vote for the feature if you are looking for separate customfields:

That being said, you may find plugins that have this functionality in the marketplace.
