Cannot install GitHub for Atlassian - "Connect" button does nothing

I have followed the set up instructions for connecting GitHub to Jira. I am using GitHub enterprise (my URL is xxxx.ghe.com), I did automatic app creation, I installed the app to my organization, I installed it to an organization for all repositories, I got to the github.atlassian.com screen where I choose the Jira site and I can see my Jira site but when I click "Connect," nothing happens.

How do I get Jira connected to my GitHub?

After this, the app is installed in my organization, but remains disconnected from Jira. If I go back to the "GitHub for Atlassian" application in Jira and click through the setup process, I get back to that same github.atlassian.com screen but now it says "No GitHub organizations with Jira installed."

I have deleted the apps from both sides and gone through this process 3 times now.
