Use Case: Capturing Comments in Jira Cloud

I want to capture comments from users that include "7 Questions." Specifically, my questions are formatted in Yes/No style, and I would like to store both the questions and their corresponding answers in one centralized location.

To achieve this, I am considering using a text field to compile all relevant comments and responses. If there are alternative methods for organizing this information more effectively, I would appreciate any suggestions.

Rather using the comments, I would suggest to create a service management project where in the request type you can specify forms that you can create either from a template or start from the blank.
