Configuring Okta and SAML

I have done this before, but can't get this to work again.  I have followed the instructions in this article excluding adding the app  https://confluence.atlassian.com/jirakb/how-to-configure-jira-software-data-center-for-saml-2-0-sso-with-okta-1005783953.html.  Even with the app installed previously, I get the same error.  I'm not sure what step I'm missing to have it say "Something went wrong" with the error identifier 6f6ff5a6-d5b3-4114-8e3a-b5d78c122f94. Suggestions?  I had to rebuild the server from scratch and import data from another server.  This is an on-premise server.
