Create issue on transition is not firing

I created logic for creating an issue on a transition awhile back. One day, it stopped working. Below is how I have it setup. After reading other posts on this issue, i have confirmed the order of steps 1-5 appears to be correct. What else might the issue be?

Thanks Steven, so I am still experiencing this issue, any ideas on how to address it? I think the regex was removed, but I am not sure what regex to add? Essentially i want the "create issue" to fire every time the ticket goes to the status closed.
