Create sub task screen not populating field tab

I've created a subtask screen that is configured to customize the 'Create New screen'. The new screen includes two field tabs as well as all of the custom fields. However, when I click on the Create Subtask Screen, it displays all of the fields in the same screen, but the field tab is missing. For the most part, all of the fields are displayed on a single long screen. Despite the fact that the Create New screen  has two tabs, I am unable to see them. Please assist with this matter.

Please check the configuration

Screen --> Screen scheme --> Issue type screen scheme

Screens define which field should appear and the tabs

Screen schemes define the operations --> Create, edit and view

Issue type screen scheme defines the mapping of screen scheme to issue types in project

To tag this, when creating a subtask on the top right you should see a "Configure fields" dropdown and clicking on that should show you another screen where you can configure what fields you want to see. The most helpful part though is the link that says "Where is my field?". If you click on that you can search for the field you want and it will tell you if it is available or not. If it is not it will tell you what you need to update to get the field there.
