Creating child tasks for epic based on a field condition

I am trying to create a rule that works like this:

-when EPIC is moved in progress

-IF a custom field "Medical validation only task" is "Da", then, for each file added creates another task of a certain type (Medical validation only)

-ELSE - if this field is not populated (it's a checklist field that only contains the "Da" value)

 

then, for each file added creates another task of another type than the first (Writing/validating task)

I am new to Jira and tried to implement what I read online, at one time it worked, when I didn't have the field condition and I was only creating Writing/validating tasks. Now I modified the rule - didn't change the "file" smartvalue rule, and the automation fails - no tasks are created and I get the log:

Hi @Alexandra Peia 

Please provide images of your advanced branch details and of the audit log details showing the rule execution.  Those will provide context to help explain the symptom.

Also, what was the specific change you made to the rule, after which it stopped working as expected?
