Creating subtask on transition with fields from transition screen?

So I have the post function plugin that creates a subtask on a transition, but it inherits all of it's fields from the parent. There are a few fields on the subtask that are not in the parent issue, so I added the subtask creation screen to the same transition, but it does not copy over. How would I add values on the transition screen that will show up on the subtask details?

You'll need to have a post function copying your sub-task fields to your parent-ticket. You need this add-on for that.
