Is there any way to report or view the original due date for an action item once the due date has been changed? Or does this have to be added as a custom field and manually entered?

You can view the original due date value in the History tab in the Issue detail view.
Hi Paul,
If you want to keep track on the first value placed in a field you can:
1-View the history log on the view screen
2- copy the original value in to a hidden field via scriptrunner for example and let user change the original value in the first field if they want
