We are hosting our own Confluence on-prem and would to know if database will be altered and affected during upgrade process.
It will change your data so that it fits and works with the new version.
You shouldn't need to care about it. Take a backup of the database before you attempt an upgrade, test it when it completes. You do not need to be looking in it, the upgrade code handles all the changes required without humans getting involved.
I had an upgrade 2 times and 2nd time failed to load Confluence because buildnumer in database and confluence.cfg.xml are difference so I was wondering what effect would upgrade have and if there is major in change in database structure.

Ok, you are looking at the wrong thing. You still should not care about the changes.
The clash in build numbers simply tells you that your upgrade went wrong and was not completed.
It doesn't matter what the changes were, they've gone wrong. You'll need to go back to the backup of the previous version and either use that, or try to upgrade it again and check that it works.

Assume that it will because it is an upgrade. May I ask why it matters? May I ask why you are speaking about self hosted Confluence, but the Deployment type / Product Plan / Tags all say Jira Cloud? Cheers!
