Email this issue bug

We are using the Email this issue Add-on and it for certain projects is is not possible to add new notifications. When clickling the Add button, nothing happens. 

There are also Issue Event Notifications active, which are not displayed in the list.

Does someone know what might cause this problem and how to fix it?
