Error when adding attachment in Create Issue form

We upgraded our Jira server from Jira 8.13.10 to 8.19.0 yesterday.

Now, when we try to create a task (by clicking on the 'Create' button on the dashboard), the icon for attachment is missing from the Description field of Create Issue form.

When we try to create a sub-task, the icon for attachment is present. But, when we attached an image, a message is shown as 'There is 1 error in this form.' (But, the image was attached successfully.)

Are these existing issues?

Is there any fix for this?

There is a Bug reported the same kind from Atlassian. 

Please try by attachment field on the screens.
