Form per issues type

Hi, in our project we have 3 issues types "new design", "revision" and "technical support".

When I create a new form it says in the top left corner "new drawing" with the following explanation:

"Receive different types of work. Each type will have different form fields depending on what kind of information you want to collect".

What I want to do is only create "New drawing" issues from this form but it lets the user select any of the 3 types even when Jira provides this explanation. Each issue type has the same fields and when editing the "revision" form, also the fields from the other issue types are removed...

Any help on this?
