Forms Not Populating Accurately

I created a new project and form with required fields, but when creating a new work item from the Kanban Board (Projects Board) I am having the following issues:

1. If a new task is created from the Project Board using the +Create option, the project form is not populating. Instead the submitter is prompted to type in their request in the "What need to be done? pop-up box without completing the form.

2. If a new task is created from the Project Board using the "Create a work item" option, the new task form displays, but none of the fields reflect as "required - (*), however when reviewing the form, all the required fields display as such.

Hi @McCoo_ Jacquie,

Welcome to Atlassian Community!

What you see is expected. Forms are intended for external users outside of your immediate team. On the work item layout that you see when clicking on + Create you cannot rename standard fields like summary and description, which can be done using a form. If you want to have required fields on the "internal" work item screen you can do that by either set validators in the workflow or via field configuration.
