Help on to open Bug Screen when any Test case Fail.

Hi,

I am using JIRA as TCM in my current project.

JIRA As TCM (Test case management)

My Query:

How i open "Bug" Screen on Click of "Fail" button in any of the Test cas.

OR

can i design screen similar to Bug screen when My Test case is fail that Consist of following fields:

-Bug Summary
-Priority
-Components
-Affects Versions
-Fix Versions
-Assignee
-Reporter
-Description
-Browse option (For Attachment)

Please help
