Branden has covered all the stuff you should do to get your fields right. However, it won't put the fields in sections, they will appear as part of the "custom field" panel.
To get them into sections, you'll need code - create some "web fragments" to contain them.

From the screenshot you sent it looks like those are multi line custom fields. To add a custom field:
Once you do that, you'll be prompted to determine which screens you want that field to show up on. Next, you'll need to configure the screen. You can choose the screen and then go to System Administration > Issues > Screens and choose Configure next to the screen to place the field where you would like it to go.
