How can I create a template for a bug?

I want that all bugs in the project will be opened in the same format and include the same details (not specifics).

For example: found on: [iOS]

Version number: [3.44]

Device: [Apple 8]

etc.

How can I do that?

Define a screen with all the fields that you would like to capture.

Then associate the screen with the issue type BUG.

Then the form that is displayed for bug across the project will be uniform.
