Is it a given that having Jira and Confluence, we are on Atlassian Cloud or is it a separate subscription? We would like to integrate Testrail with Jira and Jira Cloud or Atlassian Cloud is required.

For Jira and for Confluence your company has two options for how to use the products:
You make this choice for each product. You could have both running under the SaaS model, both running under the self-hosted model, or one as SaaS and the other as self-hosted.
One way you can tell which model your products are under is to click on the Help button in each product, in the upper right corner near your avatar.
After clicking on that you should find an "About" option. If you click on that you should see version information about the product.
For self-hosted products you will see a version number in the format ##.##.##, like this:
Cloud products do not have a version number in that form.
When evaluating integrations you will need to know your hosting model (SaaS or self-hosted) so that you can determine if the integration is supported for your environment.
