I have a project with 4 issue types : epic, story, task and bug.
I would like to add a new issue type named documentation and then customize the workflow for that issue type to be a subset of the workflow for the other pre-existing issue types.
I tried to follow the instructions at this page but did not have a whole lot of success.

"did not have a whole lot of success" does not help us understand the problem. Where exactly are you stuck on this?
Are you definitely in a company-managed project? Or are you stuck on creating the issue-type scheme? Or getting errors when editing it? Or something else?

I need simple instructions on how to add Documentation as a new issue type that will show up when creating a new JIRA issue. I tried to follow the instructions as per above link but that created a new scheme rather than adding Documentation to the default scheme where Epic, Story and Sub-Task already exist.

Documentation shows as a new scheme and does not show up when I click on Create menu button.

I am really sorry, I do not understand "documentation shows as a new scheme"?
Maybe try a different order - go to the project you want the new issue type in, go to project settings and find issue types. Select edit to go into editing the scheme and you'll find you can add/remove issue types from the global list and create new types from the "add" button.

Documentation as an issue type shows up under the workflow for the project and is also associated with the scheme for issue but Documentation as an issue type does not show up when I click Create button. How can this be solved ?

Ok, I suspect you have created "documentation" as a sub-task type.
These can't appear on the "create" drop-down, you can only create a sub-task from the issue that it will be a part of.

Ok, that is it. I can either create a sub-task or a documentation issue type from say under a story. Thank you.
