How do I configure a sub-task create screen?

I want my sub-task create screen to differ from the parent task create screen. How do I do that? Is it a matter of creating a unique screen and including it in a screen scheme that comprises both parent and sub-task screens?

You've pretty much answered your own question - create a new screen, use it in a new screen scheme, and then set up the issue type screen scheme to say "for parent issue, use old screen scheme, and for subtasks, use the new one"
