I've made a checklist template that automatically appears when you create/view a new issue in Service Desk, however when it transitions from 'New' to 'In progress' or other status - it disappears.
Is it possible to ensure it stays on the screen (including anything that has been checked in a previous status) throughout the workflow?

Can you let us know how did you create the checklist? Did you use some built-in Jira functionality or third-party app?

I just created it using the option available in Jira (when you open up the individual issue).
