How is velocity calculated?

In the release burndown charts, a velocity per sprint is given.  Velocity is based on the number of stories you've completed over the last three sprints.  

My question is how does JIRA actually define "completed"?  Is it based on:

• The JIRA ticket status has moved to "closed" (or similar)
• The JIRA resolution no longer equals Unresolved
• The ticket has been moved into the active sprint 'Done' column (which is further customized by status)

Completed is when the ticket moves to the last column on your Scrum board. Status or Resolution doesn't matter.
