How to change the display of Custom Fileds?

I am creating a custom Screen for Checklist to be used in a transition. I am using a certain checklist, radio button and free form text fileds. I would like to label to be on top of the Form Field. How do I configure that in Jira?

In the page where you add fields to the screen, you can drag and drop the fields to display them in the desired order.
